What is the first step in starting physical or aqua therapy at Elite?
Please be sure to talk to your doctor if you would like to start physical therapy or aqua therapy. You will need a prescription or an order for therapy.
We have outlined some important steps for you and if you have questions, please contact us!

After you have your order, or prescription for physical therapy, please call our office and book an appointment for an evaluation. |

At the time of your appointment it will be necessary to bring your completed paperwork along with your insurance card/s, prescription from the doctor and license for ID purposes. (A full detailed list of important information will be provided once you make your appointment. You can also scroll to the bottom of this page for a list.) |

During your evaluation, your therapist will determine how many times per week you will need to come in for treatment and what your plan of care will be. |

Immediately following your evaluation, you will set up 2 weeks worth of appointments. |

A receptionist is available daily from 7:30am-6pm (Monday-Thursday) and Friday 7:30am-4pm. Our office is closed daily from 12:15-1pm for lunch. |
