What is the first step in starting physical or aqua therapy at Elite?

Please be sure to talk to your doctor if you would like to start physical therapy or aqua therapy. You will need a prescription or an order for therapy.

We have outlined some important steps for you and if you have questions, please contact us!

  After you have your order, or prescription for physical therapy, please call our office and book an appointment for an evaluation.
  At the time of your appointment it will be necessary to bring your completed paperwork along with your insurance card/s, prescription from the doctor and license for ID purposes. (A full detailed list of important information will be provided once you make your appointment. You can also scroll to the bottom of this page for a list.)
  During your evaluation, your therapist will determine how many times per week you will need to come in for treatment and what your plan of care will be.
  Immediately following your evaluation, you will set up 2 weeks worth of appointments.

  What should I bring to my first appointment?

Completed Patient Information Forms. These can either be emailed/faxed/mailed (or can be downloaded from this website) to you once your appointment is set, or you will need to arrive 20 minutes early to fill out the paperwork in our office.

Prescription from your doctor. If your prescription is being faxed, please call our office 24 hours before your evaluation to ensure we have it.

Insurance Card(s)

License (For identification and insurance verification)

Operative report or MRI reports. This item applies to patients with recent MRI, X-RAY or POST OPERATIVE patients only.  Please provide the operative report relating to your therapy appointment.
  Will I be getting into the pool at my initial evaluation if my doctor has prescribed aqua therapy?

You will not go into the pool at your initial evaluation. Please wear something comfortable.
  Will my insurance pay for physical therapy?

At Elite Physical Therapy we are contracted with all the major health insurance companies. Each patient’s plan with that insurance company can vary, so it’s important to contact your insurance carrier to find out what they will cover. You MAY/MAY NOT HAVE a co-payment, deductible or percentage that is due at each visit. This is determined by your insurance company.

  What should I do if I feel better or worse from physical therapy?

It is important to communicate with your therapist through out the course of your treatment. If you are feeling better or worse be sure to communicate that to your therapist, rather then discontinuing treatment.
  What if I need to cancel scheduled visits?

Because your therapist maps out a plan of care based on frequency, or how many times per week they want you to receive treatment, is it important to make all your scheduled visits weekly. If you need to cancel, please reschedule that appointment within the same week to achieve optimum results.

A receptionist is available daily from 7:30am-6pm (Monday-Thursday) and Friday 7:30am-4pm. Our office is closed daily from 12:15-1pm for lunch.
